Booking and Purchase Policy for the treatment in LLC "Medical And Preventive Treatment Institution “Сlinic Kivach”  

Terms of the acquisition and pre-booking trips to the clinic are:

• For a previous appointment, you must call the reservation service: (8142) 733-533, 78-24-63, 76-61-32, or send a written request (by fax, email, or ICQ);
• In reliance on information form reservation managers, select the name of treatment program, the type of room, date of arrival the duration of treatment;
• In the presence of availability the “Clinic” confirms receipt and acceptance of the application and sends a "Customer" an invoice for the advance payment within a day by fax or e-mail;
• Against payment the "Clinic" sends a "Customer" confirmation of his reservation;
• For one calendar month prior to arrival of a “Customer" representative of the “Clinics” reports orally on the phone the final price and sends a "Customer” final invoice (exclusive of advance payment) and the Agreement by fax or e-mail. By accepting the Agreement a "Customer" shall pay the final invoice and send to the "Clinics" signed Agreement within seven calendar days;
• In the event of non-receipt any copies of the Agreement and confirmation of payment the final invoice the “Clinic” takes your reservations off;
• Advance payment is returned to the “Client” if applied;

The application is considered registered only after receipt of the advance payment to the Clinic’s bank account.

An Invoice is an integral part of the Agreement on the provision of medical services.

The fact of the final invoice payment and signing of the Agreement, signed by the Clinic for the treatment program, is a fact of the consent of the payer to the terms of the Agreement on the provision of medical services.
